Offers an Anticoagulation Clinic to veterans to assist with management of Warfarin, Dabigatran, Rivaroxaban, or Apixaban. Also provides a Diabetes Control Clinic that is offered by phone or face-to-face. Helps veterans with diet needs and gives lifestyle counseling. Will assist with medication management to improve glucose levels.

Participants set their own weekly goals as they learn and practice self-management skills to improve their self-care and quality of life. Workshops meet for six (6) weekly sessions and include: Chronic Disease Self-Management, Diabetes Self-Management, Chronic Pain Self-Management, and Building Better Caregivers.
