Providing Diabetes Management Clinics, Diabetes Screening, Disease/Disability Information for Diabetes one-on-one sessions with nurse educator and dietitian to help design a personalized, self-management training program diabetes care.

Provides Certified Nurse Midwives to offer pregnancy, post-partum and miscarriage care, with the goal of improving overall health. Midwives attend births at Fairview Hospital to provide Birth and Doula support. Midwives also provide: pelvic exams, PAP tests, breast exams and scheduling of routine mammograms, family planning including birth control and emergency contraception, treatment of other concerns like yeast infections, sexually transmitted diseases, irregular periods, PMS, sexual health, menopause care and support for women dealing with depression and partner/domestic violence, a nurse advice line, 24 hour provider coverage, breastfeeding and lactation support and consultation, free pregnancy tests, and same day and evening appointments.

Provide Pack N Play portable cribs and Safe Sleep Survival Kits to families and caregivers that are unable to provide a safe sleep environment due to financial hardship. Survival Kit contains a sheet for the Pack N Play, a sleep sack for the infant, a bedtime storybook and educational materials for the parents. Family must come to Board of Health office to pick up Pack N Play and receive instruction on proper assembly and use before taking Pack N Play home.

Class is taught by a registered dietitian and certified diabetes educator. Staff of the organization work with participants on exercise principles.

Provides a variety of services that cover women's health concerns, including screening for bone density (osteoporosis), breast exams, pap smears, obstetrics/gynecological services, pregnancy testing, birth control, midwifery, and prenatal care. Testing and treatment for sexually transmitted diseases is also offered. Also offers mammograms for men.

Provides hemodialysis treatment, training for home dialysis, pre-dialysis education, dietician, social worker, and doctors who track patients' progress with individual meetings once a week; patients also receive quarterly care conferences.
