Provides participants with a structured day of activities in a safe environment; breakfast, lunch and snacks are provided; for persons with Alzheimer's, a related disorder, or frail elderly. Transportation available.

Provides a variety of services that cover women's health concerns, including screening for bone density (osteoporosis), breast exams, pap smears, obstetrics/gynecological services, pregnancy testing, birth control, midwifery, and prenatal care. Testing and treatment for sexually transmitted diseases is also offered. Also offers mammograms for men.

Provides non-surgical gynecologic care for adolescents, contraceptive counseling, emergency contraception, birth control, pregnancy testing, sexually transmitted infection testing and treatment and HIV testing.

Completes screening, testing and follow up, identifying high risk factors that would lead to a recommendation of a Mantoux TB test, available at the weekly clinic.

Provides Certified Nurse Midwives to offer pregnancy, post-partum and miscarriage care, with the goal of improving overall health. Midwives attend births at Fairview Hospital to provide Birth and Doula support. Midwives also provide: pelvic exams, PAP tests, breast exams and scheduling of routine mammograms, family planning including birth control and emergency contraception, treatment of other concerns like yeast infections, sexually transmitted diseases, irregular periods, PMS, sexual health, menopause care and support for women dealing with depression and partner/domestic violence, a nurse advice line, 24 hour provider coverage, breastfeeding and lactation support and consultation, free pregnancy tests, and same day and evening appointments.

Free six week community workshop series to help participants learn proven stratigies to manage chronic health conditions. Workshops are: Chronic disease self-managment and Diabetes self-management. Trainings also offered to prepare volunteer trainers to lead community groups.

Grady emergency room staff provides specialized medical care for adults and/or children who have been sexually assaulted. Children maybe sent to Nationwide Children's Hospital for further specialized care if needed. Treatment includes an examination for internal and external injuries, collection of physical evidence of the assault, and preventive care and/or treatment in case of venereal disease or pregnancy.

Provide Pack N Play portable cribs and Safe Sleep Survival Kits to families and caregivers that are unable to provide a safe sleep environment due to financial hardship. Survival Kit contains a sheet for the Pack N Play, a sleep sack for the infant, a bedtime storybook and educational materials for the parents. Family must come to Board of Health office to pick up Pack N Play and receive instruction on proper assembly and use before taking Pack N Play home.

Offers anonymous HIV testing, screening, and pre and post-test counseling. Provides medical treatment, information and referrals to community resources, and mental health services to persons infected with HIV who are not yet showing symptoms of AIDS. Can work with individuals to secure medications through patient assistance or insurance programs. Provides case management services to HIV patients. Also offers HIV prevention education for groups.

Offers full obstetrical and gynecological services to patients, birth control, emergency contraceptives, pregnancy testing, STI testing and treatment and HIV testing.

Offers confidential, free, forensic examinations conducted by a nurse who is specially trained in providing care to survivors of domestic/intimate partner violence. Examinations may include documenting a history of physical/psychological abuse, assessing and documenting current injuries, and collecting forensic evidence. Nurses also provide guidance to further sources of physical, psychological, and legal assistance.

Provides free and anonymous and confidential HIV testing with same day results. STI testing is also available- results must be sent to lab so will not be provided on the same day.

Offers medical service, including: - Primary care. - HIV prevention medication (PrEP/PEP). - Gender affirming care. - Trans health. - HIV/AIDS specialty care. If gender-affirming hormone treatment is part of your care plan, staff can help you coordinate your treatment with any other medications you are taking. Staff can also teach you how to give yourself injections and answer any other questions you may have.

Provides small-group interactive evidence-based workshops for older adults with one or more long-lasting health condition, and for caregivers. Participants set their own weekly goals as they learn and practice self-management skills to improve their self-care and quality of life. Workshops meet for six (6) weekly sessions and include: Chronic Disease Self-Management, Diabetes Self-Management, Chronic Pain Self-Management, and Building Better Caregivers. Rocking Horse Community Health Center provides primary and chronic health care for families including diabetes management, referrals to specialists, a prescription assistance program and mental health screenings. Behavioral Health and Counseling available.

North Columbus Health Center offers the following services: reproductive/sexual health check-up, erectile dysfunction education, erectile dysfunction referrals, jock itch - exam and treatment , premature ejaculation education , premature ejaculation referrals , urinary tract infections – testing and treatment. We also offer: STI testing and treatment , HPV treatment , Genital wart treatment and Vasectomy referrals.

Health screenings, health information, health education, health referrals. All open to the community. Blood pressure, blood sugar, bone density, peripheral vascular disease, advance directive, hearing screenings, grief support, various lab screenings every odd number month for $10.
