Provides family practice medical care, women's health care, pediatrics, beavioral health/counseling, dietetics and nutrition and chronic care management.

Clinic provides primary health care to persons, ages 18-64, who do not have a family physician and/or are low-income without medical insurance. If you have a medical emergency when the clinic is closed, please go to Licking Memorial Hospital. If you have a dental emergency, please call 2-1-1. We do not treat pain, women's health or STDs. There is a dental clinic in Zanesville that we can refer you to.

Provides ongoing primary care, sports and work physicals, childhood immunizations, annual exams for men and women, disease and condition screenings, prenatal care and sick care for adults, children and infants. Dental, Nutrition, Spiritual, Substance Use Disorder Treatment, Behavioral Health, Vision and Pharmacy care are available for Primary Care patients. Referrals can be made for specialty care and specialty pharmacy.

Nonprofit outpatient facility that provide basic health care which may include gender affirming care, physical examinations, immunizations,nutrition assistance and diagnosis and treatment of common ailments. *Telehealth options are available.

Provides medical care for the family, treatment for Illness and Injuries, management of chronic disease, physicals and immunization, well child checkups, Obstetrics and women’s health, and specialist referrals.

Provides care for minor non-life threatening medical conditions and injuries.

Provides primary and pediatric care. Services include physical exams, screenings, podiatry, physical therapy, well-child care, flu vaccines, chronic disease management, women's health, and other related primary care services. Clients can fill prescriptions written by agency doctors at a specific pharmacy at a discounted price. Care Coordinators also assist patients with finding and applying for prescription assistance programs. Provides substance abuse and behavioral health counseling.

Offers a free walk-in medical clinic for people of all ages for diagnosis and treatment of minor illnesses only. No lab tests, x-rays or injections. Does not provide ongoing care, but works to connect patients with primary care sources. Depending on staffing, 10-20 patients will be seen. Basic work physicals can be scheduled by calling prior to coming. No sports physicals.

Provides free basic medical services including blood pressure and sugar monitoring and treatment of common illnesses. Does not provide physicals.

Offers general health evaluations and examinations. Services included screening tests, nutrition education, counseling and health and fitness education.

Offers an integrated medical clinic to provide both physical and mental health care services needed for adults residing in Summit County. Accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) as a Health Home. In addition to primary care, the clinic also provides podiatry services and dental services.

The Noor Community Clinic doors are open to patients from all faiths and cultures and not just to Muslim patients. Primary care is provided for patients who lack access to more traditional health care providers. We treat patients for chronic illness such as diabetes and hypertension, as well as more acute conditions such as respiratory infections. In the event that a patient’s condition is beyond the scope of our resources, we refer patients to the Wexner Medical Center at the Ohio State University for further care.
