Professional educators provide community programs and screenings to promote health and wellness. Programs include Women in Touch, Healthier at Home, Resource Mothers, Fatherhood Support, Centering Pregnancy, Hispanic Health, Stepping Out, Prescription Assistance and community health events for men and women. Screenings include blood sugar, cholesterol, blood pressure, osteoporosis, body fat, balance, and carbon monoxide.

Provides primary, preventative, diagnostic care, as well as pediatrics, childhood immunizations, and physical examinations for sports and employment.

Provides medical examinations; services also include testing for sickle cell anemia and HIV/AIDS.

Free medical ministry that serves uninsured and underinsured members of our community with free medical appointments, prescription assistance, and specialist appointments. Medical services include physicals, high blood pressure, diabetes, UTIs, strep throat. The clinic doesn't prescribe narcotics or currently not able to test for COVID-19. Select Lab tests available onsite.

Provides quality health care for the entire family, including midwifery, comprehensive women's health, behavioral health, help by phone, support groups for patients, and health and wellness education. Helps clients find patient assistance programs and assists in filling out paperwork.

The Heritage Community Clinic offers free primary and specialty health care to qualifying adults ages 18-64 in southeast Ohio counties. We currently offer medical clinics in the following specialties: primary care, diabetes and osteopathic manipulative medicine. Referrals are provided to other health care providers as needed, as well as assistance in obtaining medications and supplies for qualifying patients.

Serves patients with any general medical ailment such as diabetes, high blood pressure, depression, abdominal pain, emphysema, asthma, headaches, sore throat, cough, and can also perform general physical exams. Also provides respiratory therapy referrals. Diagnostic testing provided for blood tests. Limited cancer screenings provided. Lab services now offered on site. If the patients's needs cannot be met, clinic will try to arrange for an appropriate referral. Does not do PAP tests; for those, clinic recommends going to the Kno-Ho-Co Clinic. Social Services Director/Wellness Coordinator is available to patients to provide resource referral and coordination to address different needs. * All services have a faith-based message.

Also provides a Marketplace Certified Application Counselor on-site for health insurnace application assistance. Other services available include behavioral health services, licensed social worker, and pharmacy services for established patients

Family practice clinic.

Provides routine medical care and treatment for non-emergency illness. Services include physicals, lab work and referrals to speciality care. Offers screening for diabetes, high blood pressure and STDs. Does not provide immunizations, TB screenings, lab work, x-rays or prenatal care. Staffed by Muslim physicians and nurses from the central Ohio community.

Provides ongoing primary care, sports and work physicals, childhood immunizations, annual exams for men and women, disease and condition screenings, prenatal care and sick care for adults, children and infants. Dental, Nutrition, Spiritual, Substance Use Disorder Treatment, Behavioral Health, Vision and Pharmacy care are available for Primary Care patients. Referrals can be made for specialty care and specialty pharmacy.

A community clinic that provides Family and Community Medicine, Pediatrics, and Podiatry. Also helps the patient to access the 340b program and drug company assistance.

Rocking Horse Community Health Center addresses the community's primary care and acute health care needs. Also provides therapeutic services for children and families. On site staffing for Medicaid eligibility.

Services include physical exams, screenings, podiatry, physical therapy, well-child care, flu vaccines, chronic disease management, women's health, and other related primary care services. Clients can fill prescriptions written by agency doctors at a specific pharmacy at a discounted price. Care Coordinators also assist patients with finding and applying for prescription assistance programs. Provides substance abuse and behavioral health counseling.
