Provides home consultations and preventative care, home safety assessments, social services referrals, education, and assistance with health care system navigation. The program will track participants’ progress to monitor improvements.

The Triple S Wellness Program provides group/individual activities that give the client(s) the opportunity to discover hobbies they enjoy, reintegrate back into the community and interact/build relationships with others in recovery while promoting self-growth and independence.

Center of Hope is a community service center and food hub. The Bladensburg location houses MidOhio Market, which is a free grocery market and operates as a food distribution hub for mobile programs that operate in rural Ohio. Center of Hope also provides holistic supports for individuals, including chaplain services, health/wellness programming, senior services and more. We also operate a limited number of deliveries to homebound seniors.

A Community Health Worker assists older individuals to access community resources. To maintain their health, wellbeing, to advocate to live in the community safely, and encouragement to participate. The CHW stays in touch by phone, visits in the home, and in group settings providing assistance to access immunizations, health screenings, transportation to medical appointments, resources to live independently, and assistance to manage overall wellness by addressing social determinants.
