Provides individual nutrition consultations as well as assessments for issues related to cancer provided by a registered dietitian.

In-home client assessments link individuals with appropriate long-term services; services are an alternative to nursing home placement which include but are not limited to adult day care, home-delivered meals, chore service, homemaker, minor home modifications, medical transportation, personal care, social work counseling, nutritional counseling, emergency response, independent living assistance, medical supplies and medical equipment.

Provides free basic medical services for low income adults without healthcare coverage. Does not provide school/work/sports physicals. Does not provide any disability, workers' comp or social security claims paperwork. Can provide referrals to primary care specialists or specialists who offer services on a sliding fee scale, as well as dental referrals. Can arrange for lab work or radiology services. Physical therapy services available. Offers limited eye screenings.

Provides comprehensive primary care and specialty care. Pediatrics, women's health, dental. Adult transitional sickle cell, infectious disease, hematology/oncology services. Dietician and social worker, diabetes management program, lab and pharmacy.

Provide perinatal mental health counseling. Services include depression, anxiety and mood disorder during and after pregnancy, adjustment difficulties, postpartum stress and birth trauma, treatment for substance abuse and addiction during and after pregnancy through RISE Recovery, and coping with miscarriage and loss.

Provides ongoing primary care, sports and work physicals, childhood immunizations, annual exams for men and women, disease and condition screenings, prenatal care and sick care for adults, children and infants. Dental, Nutrition, Spiritual, Substance Use Disorder Treatment, Behavioral Health, Vision and Pharmacy care are available for Primary Care patients. Referrals can be made for specialty care and specialty pharmacy.

Supplemental nutrition education program for pregnant women, women who recently had a baby, breastfeeding women, infants, and children up to 5 years old. Offers the following services: * Nutrition education and support * Breastfeeding information and support * Peer support during pregnancy and while breastfeeding * Infant feeding classes * Referrals to healthcare and other assistance programs * Supplemental foods

Provides registered dietitians who can guide individuals and their families in making changes to their diet that may be required by health conditions or other circumstances. Also provides comprehensive nutrition education.

Provides support services and help to enable families to care for family members with developmental disabilities who are living at home. Services include: respite care, adaptive equipment, home modifications to accommodate the family member with disabilities, special diets, and other services individualized to each family's needs.

Delivers primary health care in the home through a hospital-based interdisciplinary team to homebound (often bedridden) eligible veterans, whose caregivers are willing and able to assist in their care. Provides medical and nursing care, rehabilitation services, dietetic consultation, psychosocial assessment, educational services, medications, and equipment.

Provides nutrition assessment and treatment services.

Provides consultation with a registered dietitian who is also a certified diabetes educator. Service includes one-on-one teaching of meal planning, diet modification, and weight control.

Provides individualized nutritional assessment, counseling, and care planning, to promote optimal nutrition and prevent disease.

Provides skilled home health care services to residents of Montgomery County. Services include: nursing; physical, occupational, and speech therapy; nutrition; enterostomal therapy; intravenous (IV) therapy; social work; and home health aides.

Offers free individualized sessions with a Holistic Nutrition and Natural Health Practitioner to discuss healthy alternatives after recovery, food preparation, meal planning, and any other concerns towards maintaining a healthy lifestyle.
