Care Consultation is a free personalized service helping families address issues that arise from Alzheimer's disease or other related dementias that can be requested by care partners/caregivers, persons living with dementia or community professionals. An individual action plan is created that can contain disease education, a safety plan, stress reduction, and any referrals to community resources needed.

Provides geriatric health care management and comprehensive assessments, including driving assessment, for those needing to evaluate their physical, psychosocial, and functional status to determine a plan for optimum care.

Provides services to help older adults live independently. Coordinates community services. Assesses client needs for homemakers, transportation, home delivered meals, socialization, and other assistance.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Assessment involves mental, pharmaceutical, and physical evaluation of client and home environment, meetings with family members, safety recommendations, and linkage to appropriate community resources. C.A.R.E. offers assessments and evaluation by USS trained staff for services/programs at United Senior Services and referrals to agencies and support programs in the community.

Provides geriatric care management services that include consultation, evaluation and planning a course of action for older adults. Offers options and makes recommendations for care. Works with the client and his or her family members, physicians and other professionals (attorneys etc.).

- Develops a plan for the care of older adults, people with disabilities and people with chronic health issues so the clients can return to or remain in their homes. - Also serves juveniles and individuals with developmental disabilities, substance use disorders and mental health issues. - Services include assessing the client's needs, coordinating the delivery of needed services, completion of paperwork required by authorized providers, ensuring services are obtained in accordance with the case plan and monitoring progress to ensure that services are having a beneficial impact on the client.

Provides comprehensive geriatric assessment for seniors. Assessments are done by a team of health care professionals including a nurse, physician, and social worker. Clients and their families return a few weeks later for a summary conference. Offers primary care and neuropsychological and psychiatric services.

Provides medical care by a physician in the home for one-time or ongoing medical conditions. Services can include primary care services and treatment for chronic conditions, one time assessments (including geriatric assessments), assistance developing a plan of care, help arranging other needed mobile health services, This is a non-emergency service.

Provides services if enrolled in their programs, a case manager helps ensure an individuals needs are met with home and community based services. These services are delivered through various case management programs and through a network of providers and may include but not be limited to help with personal care, housekeeping, preparing or receiving home delivered meals, transportation, adult day care and home medical equipment.

Provides in-home assistance with personal care, homemaking, emergency response systems, meals on wheels as alternative to nursing home placement. Services for Older Adults includes Life Essentials, which has guardianship and intervention for seniors with mental illness.

Provides blood pressure screening and blood sugar screening.

Provides comprehensive medical care, dementia evaluation, fall assessment and management, geriatric consultation, medication evaluation, incontinence evaluation and treatment, and social services. Staffed by geriatricians, a geriatric nurse practitioner, a geriatric social worker, and a nutritionist.

Provides a detailed assessment to determine level of care needs and explanation of available services and ongoing supportive services such as monitoring and service coordination. Assistance with guardianship referral or nursing home placement available if needed.
