C.A.R.E. offers assessments and evaluation by USS trained staff for services/programs at United Senior Services and referrals to agencies and support programs in the community.

If enrolled in their programs, a case manager helps ensure an individuals needs are met with home and community based services. Provides in-home mental health and substance abuse assessments for older adults. Determines the clients' needs and will refer them to older adult programs in the community. Also completes competency assessments.
