Outpatient Care Coordination | University Hospitals Parma Medical Center

A registered nurse and social worker provide assistance to patients/families, including home evaluation, patient assessment, and recommendations for future care. May include, but is not limited to, help with nursing home placement, or education of the patient in current residence which might help prevent unnecessary ER visits or hospital stays. Services can be requested individually or combined. Can provide limited home bathroom modifications for safety. Does not include skilled nursing services. When program is full, assists those on waiting list to find needed services in the community. Chore services and limited bathroom modifications cannot be assessed without using other services.
