Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides Medicaid and state funds to assist and enable an individual with long-term care needs to remain in their own home. Services paid for by PASSPORT and provided by agencies that PASSPORT contracts with. Some of the services available under this program are: personal care, homemaker, adult day care, transportation, medical supplies, home-delivered meals, and nutrition counseling.

Provides an activities of daily living assessment for older Lakewood residents. The assessment is most often conducted in older person's home where the social worker informs the client on the scope of agency services and other community services available to support the elder in retaining independence in his/her own home for as long as possible.
Helps disabled older adults remain safe and independent at home. Prevents unnecessary nursing home placements. Consumer-Directed option gives consumers the option to serve as employer by hiring eligible individuals of their choice to provide their personal care services.

Investigation, intervention, and advocacy for older adults who are at risk or victims of abuse including physical or emotional abuse, unsafe or hazardous living conditions, exploitation, neglect, abandonment, or financial exploitation. Hotline for abused adults and concerned others who are in need of advice, guidance and/or emotional support. *APS will go to the home and complete a assessment of activities for the client and help make referrals for voluntary services to prevent, reduce, or remedy conditions causing endangerment.

Provides case management and coordination of needed home care services in the following areas: medical transportation, emergency response system, personal care, homemaker services, home delivered meals, and chore services, including laundry service. Services can be requested individually or combined. Can provide limited home bathroom modifications for safety. Does not include skilled nursing services. When program is full, assists those on waiting list to find needed services in the community. Chore services and limited bathroom modifications cannot be assessed without using other services.
