Individual Support Coordinators (ISCs) support clients through coordination of services including social, medical, education, and employment. Services may include: *Identify the person’s strengths and resources. *Determine the needs of the person and work to find resources to support those needs *Create a plan that helps individual reach their goals *Connect individual and family with resources and help coordinate between multiple agencies when needed *Check in regularly to discuss updates, changes, and successes *Family supports are also available.

Provides screening and evaluations for hearing, speech, and language, and audiological evaluations. Speech and language therapy, hearing evaluations, and hearing and speech aids vary per site.

Offers sexually transmitted disease (STI) screening for a variety of diseases. Treatment may also be available.

Provides medical care by a physician in the home for one-time or ongoing medical conditions. Services can include primary care services and treatment for chronic conditions, one time assessments (including geriatric assessments), assistance developing a plan of care, help arranging other needed mobile health services, This is a non-emergency service.

Providing sports physical to area student athletes. During the sports physical days, immunizations will also be offered at an additional charge.

Provides rapid tests to pick-up for at-home COVID-19 testing. Limited to four tests per person

Provides a way to give individuals quick access to lifesaving treatment for COVID-19. People are able to get tested and, if they are positive and treatments are appropriate for them, receive a prescription (for Paxlovid) from a health care provider, and have their prescription filled all at one location. If administered correctly, the treatment may limit the severity of a case of Covid 19.

Anonymous or confidential (whichever the client chooses) rapid testing for HIV using Oraquick collection device (no needles; no blood. Results available same day.

Group offering education, encouragement and support to anyone whose life is effected by an ostomy. Family members, and people considering an ostomy are welcome. Meets on odd months at Humility House 755 Ohltown Road, Austintown; and even months at St. E's Boardman 8401 Market Street in the Woodland Room. Parking is free and light refreshments are served.

Provides a variety of health screenings, including high blood pressure and cholesterol, diabetes, tuberculosis, and colon cancer detection. (hemoccult screening and colonoscopy included).

Provides geriatric care management services that include consultation, evaluation and planning a course of action for older adults. Offers options and makes recommendations for care. Works with the client and his or her family members, physicians and other professionals (attorneys etc.).

Provides assistance with managing chronic diseases such as heart disease, diabetes and asthma. Diabetes management includes help with preparing healthful meals that may change the course of the disease.

Provides primary and preventative health care including pediatric and adult medicine, physical examinations, asthma/allergy, treatment of sexually transmitted diseases, school and sports physicals, assistance completing applications and referrals to specialists. Services may vary per location. Lorain - Central site also offers women's health and OB/GYN services. Lorain - South site also offers podiatry. NOTE: Assistance with transportation to appointment may be available.

Provides free vision exams for income-eligible workers and their families who: have a job or live in a household with a working member, have no vision insurance, have income below an established level based on household size, and have not had an eye exam in the last two years.

Offers testicular examinations and prostate exams.

Offers a multi-specialty clinic for children who use a wheelchair and who require special adaptive equipment to improve their positioning and promote functional independence. The seating team outfits and customizes wheelchairs and other seating devices, building in growth potential to all seating systems. Service includes referrals to vendors who provide the needed seating systems.

Provides Certified Nurse Midwives to offer pregnancy, post-partum and miscarriage care, with the goal of improving overall health. Midwives attend births at Fairview Hospital to provide Birth and Doula support. Midwives also provide: pelvic exams, PAP tests, breast exams and scheduling of routine mammograms, family planning including birth control and emergency contraception, treatment of other concerns like yeast infections, sexually transmitted diseases, irregular periods, PMS, sexual health, menopause care and support for women dealing with depression and partner/domestic violence, a nurse advice line, 24 hour provider coverage, breastfeeding and lactation support and consultation, free pregnancy tests, and same day and evening appointments.

Provides registered dietitians who can guide individuals and their families in making changes to their diet that may be required by health conditions or other circumstances. Also provides comprehensive nutrition education.

Provide family planning services; free pregnancy testing. Provide testing and treatment for gonorrhea, syphilis, chlamydia, genital warts, and other STI’s. Provides information and educational presentations on sexually transmitted diseases to at-risk teen groups at community centers, Youngstown State University’s health classes and teachers, community groups, and health fairs.

Provides full fire services for the City of Bedford Heights. Offers blood pressure screening. Teaches CPR classes upon request for companies or community groups. Provides fire prevention brochures when available. Provides smoke detectors to low income residents in need of one when available.

Provides medical examinations; services also include testing for sickle cell anemia and HIV/AIDS.

Provides monthly lab screenings.

Provides screening through colonoscopy to diagnose colon cancer. When patients are high risk program takes individual family histories, provides testing through colonoscopy, treatment (if indicated) and referral to appropriate community resources for those who are (through genetics) at high risk for colorectal cancer.
