Do Not Resuscitate Or Do Not Intubate Forms | Ohio Department Of Health

Provides information about Do Not Resuscitate (DNR) orders, which relate a person's wishes not to be revived in the case of a catastrophic medical situation. A person may create a DNR order in consultation with a physician, certified nurse practitioner, clinical nurse specialist, or physician assistant.
