Accepts reports of health care providers or beneficiaries who make false statements or representations which result in an unauthorized payment by the Medicare program to themselves or another. Examples of Medicare fraud include incorrect reporting of diagnoses or procedures to maximize payments; billing for services, medical supplies, equipment or medications not provided; misrepresentation of the dates and descriptions of services or medications provided, the identity of the recipient or the individual furnishing services, etc.

The Fraud Unit's mission is to detect, prevent and act as a facilitator regarding consumer/provider insurance fraud in Ohio. The Fraud Unit is a criminal justice agency. The unit will investigate complaints of persons suspected of fraudulently attempting to receive a benefit from an insurance company. The unit will initiate criminal proceedings by presenting evidence of the commission of any criminal offense established under the laws of Ohio relating to insurance to the prosecuting attorney of any county in which the offense may be prosecuted, or to federal prosecutors.
Provides methods that the public can use to report to cases of suspected fraud involving claimants, employers, insurance company officials, officials of third-party administrators, insurance agents, lawyers, health care providers, and vocational rehabilitation providers who have lied to obtain a benefit through the Workers Compensation program or to prevent a benefit from being awarded.

Battles fraud in the health care industry through the Medicaid Fraud Control Unit and the Workers' Compensation Fraud Unit. The Medicaid Fraud Control Unit is responsible for the investigation and prosecution of health care providers accused of defrauding the state's Medicaid program. In addition, this Unit also enforces Ohio's Patient Abuse and Neglect Law, which protects the mentally and physically disabled and the elderly from neglect and abuse in Ohio's long-term care facilities. The Workers' Compensation Fraud Unit is responsible for prosecuting claimants, employers and health care providers who defraud the Ohio Workers' Compensation Program.

Accepts reports and investigates allegations of unlicensed insurance transactions, potential agent misconduct, insurance fraud and other crimes related to insurance.

Assists with prevention, detection, and reporting of suspected Medicare fraud, errors, and abuse.
Ohio Senior Medicare Patrol (SMP) trains people to prevent, recognize, and report health care fraud, errors, and abuse. SMP team members participate in outreach events to help educate Medicare beneficiaries how to protect themselves from medical identity theft. SMP staff can help if your Medicare number has been compromised.
SMP staff and volunteers educate and empower Ohio Medicare beneficiaries to recognize, avoid and report suspected Medicare fraud, waste and abuse.
Handles complaints and investigates issues dealing with suspected Medicaid fraud by providers and possible neglect or abuse of residents of long term care facilities in Ohio. Will prosecute if criminal action is found. Complaints addressed include, but not limited to, suspected Medicaid fraud by providers (billing for services not performed, over billing, kickbacks etc.) and possible neglect or abuse of residents of a long term care facility (such as nursing homes, group homes, MR/DD facilities, and hospital long term care units). Does not represent the person making the complaint as a private attorney would.
Provides a hotline the public can use to report instances where applicants, policyholders, claimants, providers, agents or insurance company employees have made false statements or otherwise intentionally misrepresented the facts during the process of buying, using, selling, or underwriting insurance.
