Provides transitional shelter for homeless men and women who are recuperating from an illness or injury. Offers one facility with one space for men and the other for women. Each room is private and residents will receive ongoing medical supervision, medication management, meals, and coordination with local health care providers. Residents also receive intensive case management that includes submitting housing applications, benefits review, assistance to file for SSDI and to resolve barriers to self-sufficiency, healthcare enrollment, and rent/utility assistance. Provides on-sight assessments for substance use and mental health disorders. Assists with referrals to obtain treatment; provides transportation to and from any needed medical or housing appointments (if not covered by insurance).

Provides a combination of transitional housing and rapid rehousing to single homeless women. Transitional facility has 15 beds, is shared living, and provides a safe environment for participants. It is staffed 24 hours a day. Provides intensive Case Management, meals and snacks, bus passes, van transportation, onsite laundry equipment and supplies, toiletries, group outings, life skills training materials and activities, Wi-Fi, and cable TV. Length of stay up to six (6) months. NOT handicapped accessible. Implements a zero tolerance for drug use. Rapid ReHousing assists participants in securing and maintaining a one-bedroom apartment located within Summit County. The program provides intensive Case Management, housing placement, security deposit, monthly rental assistance, utility assistance (one time only), bus passes, moving costs (one time only), bus passes, group outings, and other miscellaneous items. Length of participation is up to 24 months.
