Assisted Living Services provides nursing care that is always available, as well as a multitude of services such as fine dining, housekeeping, laundry, cable, planned activities, and a Chaplain on staff. The F.F. Mueller Residential Center is a State licensed and Department of Health certified Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID.) Individuals with severe developmental disabilities receive 24-hour/7 days a week nursing care and active treatment. It is located on the main campus of Developmental Disabilities of Clark County. Staff receives specialized training to work with and meet the social, psychological, medical, developmental, educational, and vocational needs of the individuals who call this their home. The FF Mueller Center is funded by Medicaid and our local levy.

Provides short-term rehabilitation, on-site therapy services, home assessments, 24-hour nursing care, wound management, peritoneal dialysis services, IV therapy, bariatric care, alzheimers and dementia care and respite care. Private and semi-private rooms, Beauty and barber salons on-site, Appointment transportation, Laundry and linen service offered.

Maintains 100 bed facility and provides minimal, intermediate, skilled and subacute nursing care for persons with Alzheimer's and other related dementials. Facility and activities specifically designed for persons with Alzheimer's disease and other related dementias.

Colonial Nursing Center is an assisted living facility.

A residential program with 7 locations throughout the County, five of which are agency owned, and all of which are agency-operated, offering single, shared and congregate living arrangements.

Provides long-term residential services for adolescents and young adults whose primary diagnosis is developmental disability, who may have other multiple handicapped conditions, and who exhibit challenging behaviors. Offers comprehensive training and support programs.

Provides temporary transitional housing with treatment and support services for Lake County adult residents who are severely mentally disabled. Programs are designed to provide a safe, supportive environment for persons who need assistance with personal care in order to gain or resume independent living situations in the community.

Offers a full range of services for individuals diagnosed with developmental disabilities population, extending from preschool to older adults. Services include education, work services and supported employment for adults, residential services, supported living services, family resource services, nursing, transportation, and case management services.

Offers custom assisted living care options so residents can receive as little or as much help as desired.

Residential Developmental Centers for the Developmentally Disabled. DCs serve people who require comprehensive programs, medical, behavioral, and residential services, and assure that an array of choices ensure that each resident may experience a life of increasing capabilities and independence. The staff support individuals through developing a person-centered plan and using Trauma Informed Care practices to identify the supports needed to achieve the individuals' goals. DCs provide temporary residential placement, offer programs that teach skills individuals need to live in a less intensive, more community-based setting. Centers also provide technical assistance and training to County Boards of DD, families and providers in order to assist an individual to remain in their own home and avoid admission to a developmental center. If the team determines the individual needs a short-term admission to assist in stabilization, the admission to a center is coordinated with the County Board in the county the individual lives. Planning for the person's return to their home begins at admission and includes the people who are important and necessary to make the individual's return to their home successful. It is also DODD's belief that all residents of its centers, with the appropriate supports and services, can live safe and enjoyable lives in their own homes in the community. Following the discharge of a person from a center into their own home, staff from DODD will provide follow-along services to the individual, county board and provider to ensure that services are being appropriately delivered and to address any situations that may arise as the individual adapts to his/her new home.

Provides 26 supported living residences for adults who have developmental disabilities. Maximum of four people per residence. Geauga County Board of Developmental Disabilities (GCBDD) provides 24-hour in-home support services through contracted providers.

Provides residential care for 90 adults who are non-ambulatory and who have profound or severe developmental disabilities. All residents are served by a team that includes a teacher, therapist, caregiver, dietitian, nurse, social worker and behavior specialist. Offers dietary assessments, nursing care, physical and occupational therapy and recreational and educational programs.

Provides residential living with comprehensive care services and educational programs for individuals with developmental disabilities. Services include instruction in self-care and daily living skills.

Provides around the clock supervision, personal care and supportive services in group home settings to severely mentally disabled men and women.

Funds and arranges support services for adults with developmental disabilities to live in group homes or in their own homes with up to three other individuals with developmental disabilities. Service providers are certified by the Ohio Department of Developmental Disabilities.
