Assists with the Medicaid process for individuals diagnosed with a severe mental illness, or have been diagnosed with one of the following chronic conditions: such as Cancer, HIV/AIDS or Immune Deficiencies, End Stage Renal Disease, Sickle Cell Anemia, Cystic Fibrosis, Hemophilia. Serves those who have had a previous transplant as well as those who are active on the solid organ or soft tissue transplant waiting list.

Same as site hours. - Agency goal is to provide approval or denial (original or reinstatement) within four to six (4-6) weeks, but in some cases, determination may take longer (not including disability determination).

Application process
- Individuals who are not already on Medicaid must be referred by their health provider. - Those already on Medicaid should contact the agency. The application process may be started online through the Ohio Benefits self-service portal, over the phone, or in person. Arrangements will then be made with agency.

Eligibility
- Serves residents of Carroll County that are a United States (U.S.) citizen. - Must have been diagnosed with a severe mental illness, or have been diagnosed with one of the following chronic conditions such as Cancer, HIV/AIDS or Immune Deficiencies, End Stage Renal Disease, Sickle Cell Anemia, Cystic Fibrosis, Hemophilia. - Serves those who have had a previous transplant as well as those who are active on the solid organ or soft tissue transplant waiting list. - Those with chronic conditions must also be age 21 and over, receive Social Security disability benefits, have one (1) or more inpatient admissions due to the chronic condition, receive a qualifying score on the Adult Needs and Strengths assessment, and reside in an approved Home and Community Based Services setting. - Individuals who are not already on Medicaid must be referred by their mental health provider. Those already on Medicaid should contact their caseworkers. - Financial eligibility is an income at 225% of the Federal Poverty Level and assets up to $2,000 for an individual, $3,000 for a couple. Some individuals may also be subject to asset limitations.
