Application process

Phone or email for information or to make a complaint. Client should provide as much detailed information about the complaint as possible such as the name and address of the facility, the date and time of the concern and identifiable resident, patient, and staff information. Depending on the call volume that the agency is receiving, hold times may be long and in some cases the caller will need to leave a voice mail. Clients who are in immediate danger for their health and safety should also be referred to appropriate crisis lines (such as a sexual assault hotline) or to the police.
