Provides primary care, dental care, and behavioral health (including medication management and counseling/talk therapy). Has a food as medicine program for those patients that are food insecure. Has Medication Assisted Treatment (MAT) program. Free transportation offered for appointments. Assistance with insurance enrollment and financial counseling. Offers telehealth appointments (phone or virtual). Also has medic box that can be taken to patients home for appointments if the patient is homebound or afraid to leave their residence.

Application process

Walk in or call to make an appointment. For Behavioral Health Medication Management, a referral is required by primary care provider. If patient has no primary care provider, services can be established with a Family Health Services provider.

Fee

Accepts Medicare, Medicaid, managed care, and private insurance. A sliding fee scale is available based on household size and income.
