- Serves residents of Carroll County and the adjacent counties of Columbiana, Harrison, Jefferson, Stark, and Tuscarawas.
- Must be at least 21 years of age.
- Must be an Ohio resident for at least 45 days.
- Must be a resident of your home county for at least 30 days.
- Must have a passport-sized color photograph taken within 30 days of application date.
