Eligibility

Student athletes needing a physical exam to play school sports.

Required documents

Students need to bring necessary paperwork provided by the school sports program. Students are also encouraged to bring their immunization record. If students are not accompanied by a parent, they should have their consent form signed by a parent prior to attending the clinic to authorize the provider to perform the exam.

Locations: Mon-Fri 8am-4:30pm at Knox Public Health - Upper Gilchrist Road - Mount Vernon. Mon-Fri 8am-4:30pm at Danville Community Health Center - 16 East Main Street - Danville.
