Application process

Contact the Program Coordinator by phone to be pre-screened and receive an application. Patients may be matched with a volunteer dentist based on their application. An applicant may be placed on a waiting list if a county has more applicants than volunteer dentists available.

Vary by service provided.

Eligibility

  • Donated/no-cost care is provided to older adults (age 65 or older), people with disabilities, and those with serious medical conditions.
  • Discounted care is provided to adults with lower incomes and/or severe dental needs who do not have dental insurance or qualify for Medicaid.
