Photo ID: accepts a variety, insurance card. For sliding scale: proof of residency and income: accepts a variety of proofs.

Application process

Assessments are required before scheduling with a therapist and are generally scheduled within three days. Phone M-Th: 8:00am-5:00pm and F: 8:00am-4:30pm to schedule. Appointments with a psychiatrist, if needed, are generally scheduled within one week of the assessment.
