Application process
Call M-F: 8:00am-5:00pm to make an appointment for an evaluation or to make a referral. Referral must be made in writing from the patient's physician. May fill out an application for financial aid also.

Sliding scale for those without insurance. Third party payment accepted, including Medicaid and private insurances. Financial aid may be available.
Eligibility
No geographic restrictions. Early assessments for those through age 3 years, 11 months. Diagnostic testing for those older than 4 years is also offered. Early intervention for those through age 5.
