Fees are negotiated with contracted employer for EAP. Normal billing procedures for any services outside of EAP. DOT-SAP service is an all-inclusive rate of $600.00 which includes: Evaluation, Recommendations, Case Management, Follow-up Evaluation (Return-to-Duty).

Description

Provides contracted services with local employers to offer confidential help to employees, and in some cases their families, whose legal, financial, marital, parent-child, childcare, substance use, health and/or mental health problems could have a direct impact on their attendance and job performance. EAPs vary in complexity from telephone hotlines that offer referrals for needed services to organizations that offer in-person diagnosis and referral, direct counseling and/or extensive treatment for one or a variety of problems. LAPP also provides Department of Transportation-Substance Abuse Professional services. This includes evaluation and recommendations for violations related to substance use for all safety sensitive positions. This ranges from CDL drivers, railroad workers, and airline employees. Once recommendations are completed, a follow-up evaluation is conducted to determine if a Return-to-Duty letter advising the employer completion was satisfactory.
