Ohio Home Care Waiver / Program | Geauga County Job And Family Services

Provides an integrated program of home care services designed to address the specific home care needs of those needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services (formerly Core Services) and Waiver Benefits. Home Health Services include nursing, daily living, and skilled therapy; Waiver services, for those whose medical condition and/or functional status would qualify them for Medicaid coverage in a nursing home or hospital, can also include home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite, social work and care coordination.

Paid through Medicaid, but there may also be some patient liability.

Eligibility

Open to residents of Geauga County who need nursing services every day because of unstable medical conditions, and to those under age 60 who would otherwise be in a nursing home because of illness or disability.
