Cards may be picked up Mon-Fri 8am-4:30pm.

Application process
Pick up card at United Way of MPM, visit www.singlecare.com/familywize and print or download a card. Present card to participating pharmacies. Call your pharmacy to verify acceptance of the SingleCare card.

Card may be used to receive up to 30% discount on prescription drugs, including refills, at the following pharmacies: Sam's Club, Rite Aid, Walgreen's, Wal-Mart, K-Mart, CVS, Kroger's, Shriver's Pharmacy, Riesbecks Pharmacy, Roseville Pharmacy, Lawler's Pharmacy, Blacker's Pharmacy, Johnstons' Pharmacy, and Target. Cards may be picked up at United Way of MPM or their partner agencies. Card can be digitally downloaded to your device by visiting the Family Wize website. The Familywize card can be used with insurance and Medicare Part D plans. In some cases the negotiated rate from your insurance may be lower than the SingleCare rate.
