Open to residents of Boone, Campbell, Kenton, and Grant Counties age 60 and older who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.

Co-payment may be required. Co-payments are set on a sliding scale based on income, assets, and medical expenses.
Offers assessment and case management for older adults to assist them with remaining in their home as long as possible. Through case management services, participants have access to a variety of in-home services, such as chore service, personal care, homemaker services, home delivered meals, and transportation assistance to medical appointments.
