Lake County adults with severe mental heatlh challenges currently in an inpatient psychiatric setting or at high risk of hospitalization.

Description

Provides temporary transitional housing with treatment and support services for Lake County adult residents who are severely mentally disabled. Programs are designed to provide a safe, supportive environment for persons who need assistance with personal care in order to gain or resume independent living situations in the community.
