Open to Medicaid recipients who have Humana as their State Medicaid Managed Care Organization (MCO) and
- are currently pregnant or have recently given birth (within the past 3 months)
- complete a comprehensive assessment and at least 1 follow-up call with a HumanaBeginnings Care Manager
