Eligibility

- Serves lower-limb amputees needing prosthetic care. - Must be a United States citizen or a permanent resident of the United States. - Must have no other means to pay for the prosthetic care including Medicaid, Medicare insurance, insurance coverage, or state assistance. - Does NOT assist with co-pays or co-insurance costs.

Required documents

Must provide a completed application including prosthetist pages, income verification for the household, and a readable copy of applicant's photo ID or passport, social security card, and birth certificate. If not a U.S. citizen, applicant must send a copy of their Permanent Resident Card or naturalization certificate.

Monday - Friday: 10:00am to 6:00pm.

Application process

Call the office or visit the website for more information and to download or complete an application. - The applicant must have a prosthetist evaluate, fill out, and sign the prosthetist page. - Apply online or mail, email, or fax the entire completed application to Limbs for Life (LFL) with the required documents. - Background checks are performed on every applicant and those with felonies will be reviewed by an advisory committee. LFL encourages applicants to pursue other resources and apply for Vocational Rehabilitation Services when available. Individuals may apply with LFL along with other options and if approved for other services, applicants are requested to notify LFL to discontinue their application.

Fees

No fee for approved applicants. Limbs for Life WILL NOT commit to paying any charges incurred before an applicant has been confirmed.
