Offers a free service where you are partnered with one community health worker who will walk alongside you to help connect you to the local resources you need to become stronger and healthier. The HUB is a regional care coordination network focused on improving health outcomes. We partner with primary care, behavioral health, social service providers, and health payers to develop a better coordinated and connected delivery network for low-income residents in our community who are at high risk of poor health outcomes.
