Phone for information and an appointment. Accepts walk ins during scheduled times, but slots are limited, so it is best to arrive at least 30 minutes before the clinic begins in the morning or begins in the afternoon.

Sliding scale (will slide to zero). Third-party payment accepted including Medicaid, Medicare. Will allow patients to use the sliding scale or private insurance. Prescription coverage will not be delayed if indigent or homeless and agency will determine indigent status using their sliding scale. Prescription cost will vary.

Description

Provides primary and pediatric care. Services include physical exams, screenings, podiatry, physical therapy, well-child care, flu vaccines, chronic disease management, women's health, and other related primary care services. Clients can fill prescriptions written by agency doctors at a specific pharmacy at a discounted price. Care Coordinators also assist patients with finding and applying for prescription assistance programs. Provides substance abuse and behavioral health counseling.
