Application process
Must fill out an application and return it with a prescription for the parking permit from a personal physician/chiropractor and with the required fee. Most efficient way to receive a placard is to walk in with completed documents to a designated location, preferably no later than 30 minutes before closing daily. If documents are in order, a red temporary placard will be issued and a permanent blue placard will be ordered. May also call during above hours to request an application by mail or may download application online. May mail completed application along with appropriate documentation and fee. Renewal applications are mailed once placards have been issued. Allow 3 weeks' processing for applications by mail.

Eligibility
Ages 16 and older, Ohio residents, with documented disabilities. Special plates may be issued to any person for having a disability that impairs or limits their ability to walk, as certifed by their health care provider. Temporary placards are issued for disabilities of six months or less. Permanent placards are for disabilties of longer than six months.
