Eligibility
Destinations must be less than 30 miles from the member's home to be eligible. Assistance is limited to 15 round-trips or 30 one-way trips total per year. One other adult may accompany a rider on their trip.
Rides may be scheduled by:
- members of Molina Medicaid age 18 or older
- parent/legal guardian/authorized representative of a Molina Medicaid member

Application process
Contact by phone at least two business days (48 hours) in advance to schedule a ride. Note that this does not apply for urgent needs (such as same-day or next-day visits). Rides may be scheduled up to 30 days in advance.
