Provides in-home services for adults 60 and older as an alternative to nursing home care. A social worker or nurse will visit the client to assess needs and create an individualized plan. Services may include adult day services, home delivered meals, personal care, medical equipment, minor home repairs/modifications, and homemaker services such as house keeping.
