Application process

Those who are ages seven and over who have insurance may walk in. Ages younger than seven, phone for more information or to make an appointment M-F: 8:30am-4:30pm.

Fees

Fixed Fee; Third-party payment; Medicaid; Medicare; and Private insurer accepted. Financial counselors are on site to assess payment based on income for those without insurance.
