Serves residents of Summit County who are deaf or hard of hearing and make less than $69,000.
- Photo ID for the tax filer (and spouse, if filing jointly).
- Social Security card, official correspondence from Social Security Administration that includes Social Security number (Form SSA-1099, and/or any form issued by SSA is a valid substitute for Social Security card), or Individual Taxpayer Identification Number (ITIN) letter are required for ALL people listed on the return.
- If anyone on the tax return has been a victim of identity theft and/or has been issued a 6-digit Identity Protection Personal Identification Number (IP PIN) by the Internal Revenue Service (IRS), bring that document to the appointment. The tax return cannot be filed without it and the IP PIN must be issued for the 2024 calendar year.
- Wage and earning statements such as W-2 / W-2G / 1099 forms including 1099-R, -MISC, -SSA, -NEC, -G or -K
- Interest and dividend statements from banks (Form 1099-INT and 1099-DIV).
- Birth dates for all listed on the return.
- If divorced, the divorce decree parent custody guide, and if required, Form 8332.
- Alimony paid and received, if divorce decree was finalized December 31st, 2018 or earlier.
- Documentation of child care expenses, and child care provider's Social Security or Employer ID Number.
- Documentation of educational expenses (such as the 1098-T Tuition Statement or student account statement from school).
- Any other relevant tax information about income and expenses, including any correspondence received from the IRS.
