Application process

  1. Confirm whether a disease is considered "reportable" by checking online reference sheet
  2. Complete an Ohio Confidential Reportable Disease Form or Animal Bite Intake Form
  3. Return completed form by fax

Eligibility

Any individual having knowledge of a case or suspected case must report certain communicable diseases to the health district. Additionally, all animal bites from animals with the ability to transmit rabies to humans must be reported for investigation.
