Certified birth records can be provided for anyone born in Ohio (Not just Knox County). Only certified copies are available, NO plain paper copies. There is a fee. Payment is due at time of request. There is a form to complete. Requests are fulfilled immediately.

Application process

There is a form to complete. Form is available onsite or on website at www.knoxhealth.com.

Fee

$25 fee per certificate. Cash, check and credit/debit cards accepted for payment.

Eligibility

Anyone can purchase a birth certificate, not just the person identified by the certificate. Regardless of what county in Ohio an individual was born, the record can be accessed and printed at Knox Health.
