Senior And Adult Services Gatekeeper Program | Summit County Public Health
Proactively works with local agencies to identify older people 60 years of age and over and adults with known or suspected Alzheimer's disease or a related dementia, who are living alone, have an intellectual developmental disability and/or have caregivers in needs of support. Once a referral is received a nurse and social worker will make a home visit to assess the situation, begin linking to community resources and provide case management support.

Anyone can refer a person to the Gatekeeper Program. The following information is needed: - Name. - Date of Birth (if known). - Address. - Phone number (if known). - Presenting problem or concerns. - Referral Source Name, Contact Information and Agency.
