55 years and older with a demonstrated need for transportation. Must live in Washington County. Must be ambulatory and able to enter and exit the vehicle with little or no assistance . Volunteer drivers can not transport individuals that will receive any kind of sedation.
Must call Monday-Friday between 9am-1pm. DO NOT leave messages on phone after hours.

Riders should call in for a phone intake evaluation with volunteer dispatchers between 9 a.m. and 1 p.m. If eligibility is determined, information about the program along with forms (release of liability, HIPAA Release form, and release form for funding reimbursement) will be mailed, faxed or emailed. After forms are signed and returned, riders may begin to schedule trips by calling in during dispatcher hours above. At least 1 week lead time is suggested.

All services are need based. Donations are accepted.

Description

Transportation services are available to people 55 years and older with a demonstrated need due to physical limitations that prevent the use of public transportation. Unlimited medical trips, local and long distance. Medical riders 60 and over must contact O'Neill Senior Center as primary, Senior Wheels as back-up. For riders 55-59, Senior Wheels is primary. One trip each month for personal errands like grocery shopping, pharmacy or enrichment. All transportation services are provided by RSVP volunteers using their own vehicle, volunteering their time. Local grants enable RSVP to provide mileage reimbursement to the drivers.
