Application process
Detailed information and forms to release or request information are available on the web site anytime. Print, complete, and mail to agency with payment and required documents. May also phone for information M-F: 7:45am-4:30pm. The adopted person may access their information by petitioning the court that granted the adoption. For adoptions after September 18, 1996, the adopted person can request the information from the registry without petitioning the court.
