Eligibility
Serves low- to moderate-income homeowners of Portage County. Home must be owner-occupied.
Required documents
Completed notarized application. Must include the following:
- Proof of homeowner's insurance (current declarations page).
- Three (3) months of current paystubs for all individuals over 18 in the household. If self-employed, past three (3) years of tax returns.
- Most current SSI, Disability, Retirement/Pension statements.
- Checking, savings, certificate of deposit, assets, and investment statements from the past three (3) months.
- Copy of deed.

Note: Returning application directly to the health district will delay the process. Incomplete applications will be set aside until all required documentation is provided. Applications that are not notarized will be returned as incomplete.

No fee to apply. Depending on income level, anywhere from 50%-100% of the cost may be covered by this program.
