Notorized application must be returned to the following address by mail or drop-box in lobby: Neighborhood Development Services, Inc. (NDS) Attention Kelley Palone 120 East Main Street Ravenna, Ohio 44266. Note: Returning application directly to the health district will delay the process. Incomplete applications will be set aside until all required documentation is provided. Applications that are not notorized will be returned as incomplete.

Fee

No fee to apply. Depending on income level, anywhere from 50%-100% of the cost may be covered by this program.
