Eligibility
Serves individuals 60 years of age or older. - Must meet income verification requirements - determined by Council on Aging. - Must not be eligible for traditional Medicaid based program, but unable to pay privately for the personal care they need. - Must have an assessment completed by a licensed Council on Aging social worker. If deemed eligible, individuals will be assigned to one of the three area home health care agencies contracted with the Council on Aging.
