Medical Case Management - Chronic Conditions - Medicaid | Molina Healthcare Of Ohio

Provides a program staffed by nurses and social workers to assist in helping with weight control and chronic diseases such as asthma, behavioral health disorders, chronic obstructive pulmonary disease (COPD), diabetes, high blood pressures, high-risk pregnancy, pain management, heart problems, or any other chronic illness. Program includes learning materials, care tips, and more. Programs for expectant mothers are included. Assistance with housing, food, utilities and safety in the home are also provided.
