Required documents
A Passenger Transportation Information Form must be completed and returned before client's first transport.
- Forms are located at City Hall, at the Old Firehouse Community Center and online at www.avonlake.org/city-services/community-transportation .
Hours
- Rides available Monday to Thursday from 8:30am to 3:00pm.
- Transportation services are canceled when Avon Lake City Schools are closed due to inclement weather. If the van is not operating because of weather or mechanical problems, passengers will be notified by telephone.

- Rides may be scheduled up to three (3) weeks in advance.
- Phone in cancellations as soon as possible. Cancellations may be phoned in at any time, day or night (voicemail messages are appreciated).
