Eligibility

Serves individuals who live in a long-term care facility in Adams, Brown, Gallia, Highland, Jackson, Lawrence, Pike, Ross, Scioto, and Vinton counties. Consumer of in-home or community-based care.

Required documents

May be required to sign consent forms or proof of legal representative status such as power of attorney or legal guardianship.

Application process

Phone M-F: 8:00am-4:30pm for information or to file a complaint. Complaints can also be filed by mail, fax, or email 24 hours a day, 7 days a week.

Advocates for long-term care consumers, including residents of nursing homes, assisted living facilities, group homes, as well as clients receiving in-home or community-based care services. Services include: complaint investigation regarding client rights violations, poor care and services, etc.; advocacy services; and general information and education.
