Serves adults with mental, physical, medical, developmental or emotional disabilities residing in Lorain and Erie counties.
Must provide previous assessments and other psychological, medical or physical history that might assist with providing appropriate programs and services for the individuals seeking enrollment.

Following the center visit, the prospective enrollee and others involved express their desires for enrollment. The Program Director and other administrators, including the Executive Director, review provided information to make a decision about enrollment.
