Visit website to fill out a "House Call Request Form" online; however, additional information will be needed for registration and will be conducted by phone with in 1-2 business days. May phone for more information and for registration. Local service is dependent on how far physicians are willing to travel (If office sites seem too far from the caller try providing the Ohio Services number).

Fixed fee. Some third-party payors accepted such as Medicare, Medicaid (group home residents only) (does not accept United Healthcare) and some private HMOs.

People 18 and older who are homebound (rely on someone else to get to and from appointments) and who are in need of a physician to provide medical care in the home. Serves Ohio.
