Medicaid - Long Term Care Services (FORMERLY NURSING HOME DEPARTMENT) | Carroll County Department Of Job And Family Services
Provides public insurance to cover medical services to those of any age who require long term care, which may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Medicaid is the payer of last resort - Medicare will be billed first if client is eligible for it.

Application process
- Call to schedule appointment. - Once application is received, client will be contacted within five (5) business days to make an appointment). - An intake interview is necessary in order to obtain service (homebound individuals may conduct the interview by phone or send an authorized representative in their place - call for details). - Clients who may be eligible for Medicare will be referred for enrollment. The application process may be started online through the Ohio Benefits self-service portal, over the phone, or in person. Arrangements will then be made with agency.

Eligibility
- Must live in Carroll County and be a United States citizen. - Serves individuals of any age who need long term care. - Financial eligibility is an income at 225% of the Federal Poverty Level and assets up to $2,000 for an individual, $3,000 for a couple, those whose income is higher must create a Qualified Income Trust (QIT).
