- Serves Tuscarawas County households with at least one child under the age of 18 (or through 19 if the child is still in high school), pregnant women (at last trimester of pregnancy), and (in certain cases) the non-custodial father or mother. - Family must have income which is at or below 200% of the Federal Poverty Level. - At least one member of the family must be a United States citizen or qualified alien. - Need must be due to disaster declared by the Governor of Ohio. - All other sources of disaster relief must have been exhausted.
- Completed application form. - Proof of address - Proof of financial need (all income which has been received by any member of the assistance group (household) during the previous 30 days will be considered gross income, both earned and unearned, will be counted verbal verification of income from the income source is permissible) proof of ALL liquid assets (verbal or written) proof of the applicant's change in circumstance leading to the current need. - Written proof that the applicant made a significant attempt to use other community services to meet the need presented. - If pregnant, need a medical statement verifying pregnancy. - In addition, if the cost of the documented need exceeds the amount of PRC that is to be issued, the applicant must provide verification that they have paid the amount in excess of the PRC payment, prior to receiving payment.
