Serves residents of wards 16 and 17 (parts of ZIP codes 44111 and 44135, part of ward 11), bordered on the north by the City of Lakewood, on the west by I-71, the cities of Rocky River and Fairview Park, on the south by I-480 and Brookpark Road, and on the east by I-71, West 117th Street, West 129th Street, and West 150th Street. Serves age 65 and older or age 18 and older if disabled. Income requirements apply.
