Eligibility

On site services for adults and children. In home services for adults 18 and older who are confined to their homes and live in the following areas: Berea, Brecksville, Broadview Hts., Brooklyn, Brook Park, Cleveland (West - mainly 44135, 44142); Fairview Park, Lakewood, Middleburg Hts., North Olmsted, Parma, Parma Hts., Rocky River, Strongsville.

On site services by appointment M-F: 7:30am-8:15pm Sa: 8:00am-12:00noon. In home services by appointment M-F: 8:00am-4:30pm and possibly other hours.

Fees

Fixed fee; Third party payment accepted; Private Insurer; Medicaid; Medicare. Insurance may not cover all services. Offers the Share program to help clients understand their medical bills and set up a payment plan.
